Terms and conditions - "Smile Makeover" Instagram competition.
1. To participate in this competition, users must

c) Tag a friend in the comments to Amelia Kabath (@ameliakabath) post about Smile Makeover at American Smile competition
d) Both people need to follow @americansmile_ and @anniesmithsmiles Instagram accounts
Second Chance, Entry 2:
a) follow @americansmile_ Instagram
b) follow @anniesmithsmiles Instagram
c) re-share American Smile (@americansmile_) post about Smile Makeover competition in your Instagram stories, tagging at least one of your friends and adding a hashtag #americansmilemakeover
2. This competition will run until 23:59 on Monday 16th October. Entries made after this time will not be accepted.
3. There will be 2 winners who will be selected at random, after the competition deadline of 11.59pm on 16th October, who both have fulfilled the criteria
4. The contest is open to residents of the United Kingdom (England, Scotland, Wales and Northern Ireland) over, except for American Smile employees, all contractors and families or anyone directly connected with the organisation, and operators of its brands and concessions.
5. The prize consists of a Smile makeover for up to £5000 worth of treatment costs at American Smile with Dr.Annie Smith.
The Smile Makeover consists of:
- Free initial exam to identify if the patient is suitable for a treatment
- Free Invisalign Go (if required)
- Free composite bonding on up to 6 teeth
6. Smile makeover for the winner will be done if there are no complications to the treatment, such as teeth sensitivity, need for fillings. Any treatment, which might be required outside of this scope of work will (e.g. fillings) will need to be done by the patient prior to starting the Smile Makeover, this will not be included in the Smile Makeover prize offer.
7. All participants of the competition (if they follow @americansmile_ and @anniesmithsmiles accounts) are entitled to 5% discount on any treatment at American Smile if the treatment is booked with Dr.Annie Smith if the treatment is pre-booked until 30th November 2020. Please quote "Smile Makeover participant" at the time of booking.
8. If you and your friend are the lucky winners, you both will receive a direct message on Instagram
9. We reserve the right to provide an alternative prize of equal or greater value in the event of unforeseen circumstances.
10. Multiple entries per person will be accepted.
11. No purchase is necessary, however you will need an internet connection and access to Instagram. No refund may be claimed for expenses incurred in participating in the promotion, including in relation to the use of the internet.
12. We reserve the right to exclude any entries which we believe to be inappropriate, fraudulent or based on misconduct.
13. The prize is non-transferable, non-refundable and cannot be exchanged for any cash alternatives in whole or in part.
14. The winner will have 48 hours to claim their prize from the date that they receive a DM from American Smile account. After this time we reserve the right to select a new winner.
15. The decisions of the promoter are final and no correspondence will be entered into.
16. By taking part in this promotion, all participants consent to us collecting, storing and using their personal data submitted with their entry for the purposes of this promotion.
17. Any personal data relating to participants will be used solely in accordance with current UK data protection legislation and will not be used for marketing purposes without the individual's prior consent.
18. This promotion is in no way sponsored, endorsed or administered by, or associated with, Instagram. You are providing your information to American Smile and not to Instagram. By participating in this promotion you agree to a complete release of Instagram from any claims.
19. The promoter does not accept responsibility for entries that are lost, damaged or delayed. For example, as a result of network, system development, computer or telecommunications failures or fault of any kind - including any hardware, programming code, databases or software used in the promotion.
20. Winners may be required to participate in related publicity without further reward. E.g.: be photographed for website/newspaper stories, provide a short statement about their win.
21. The promoter reserves the right to withdraw or amend without notice this promotion in the event of any unforeseen circumstances outside of its reasonable control.
22. This promotion and these terms and conditions are governed by English law and subject to the exclusive jurisdiction of the English courts.

24. Promoter: American Smile
Once your treatment plan has been agreed with the dentist, we will provide printed details of your plan, which we ask that you read through and sign to say they have been clearly explained regarding the treatment options. If this plan changes due to radiographic or clinical findings, we will inform you and discuss this with you. Treatment plans are valid for the next 30 days from the date the treatment was prescribed.
Certain medical treatments require completion of a written consent form. We work to strict governing guidelines in order to ensure that the patient and/or legal guardian is fully aware of the treatment, aftercare and any risk before treatment is carried out.
We require a card pre-payment and deposits to secure the booking of the treatment. American Smile clinic reserves the right to make a charge for any debt passed to a debt collection agency.
Late cancellations and failure to attend appointments have a negative impact on our service and regular patients. For a deposit to be refunded American Smile clinic requires at least 48 hours notice of cancellation. Cancellations made within this timeframe, or failures to attend, are not subject to refund.
Sometimes being late for appointments is unavoidable. Please let us know if you are going to be late but please also be aware that if you are going to be more than 10 minutes late, you may be asked to reschedule your appointment. If you arrive more than 15 minutes late without our prior agreement we may consider that you have failed to attend, in which case you will not be eligible for a refund.
At American Smile clinic, our ceramic veneers, ceramic crowns, implant crowns and permanent fillings are guaranteed for a period of 12 months following placement, provided that the following conditions are met:
The client has fully paid for the treatment and does not owe the practice any money for the treatment received
The veneers, crowns & implant crowns have not been damaged as a result of an accident, trauma or excessive grinding
The client has followed all post-treatment maintenance recommendations made by our dentists.
No treatment is guaranteed for more than 1 year
Some treatments may have a guarantee of less than 1 year. In this instance, the treating clinician will inform you.
It is important you provide a full medical history and details of any medications that you take. It is also a requirement that we have your correct contact details on file. Should these change, it is very important for you to tell your dentist. While we try to keep all information updated on each routine visit, it is the patient’s responsibility to inform the clinic of any changes in either personal details and/or their medical history. All records are kept in accordance with strict Data Protection guidelines.
All patient details are stored on a secure computer system in accordance with the Data Protection Act. All clinical notes, digital radiographs, digital photographs, etc remain the property of American Smile clinic. Copies of notes, radiographs and photographs can be made available on written request. All staff undergo Data Protection training and understand the importance of information handling.
American Smile may use images and x-rays of your smile and teeth only (for marketing and educational purposes) for our website, promotional and educational literature. Your name will never be published and your identity will never be disclosed. We ensure we receive permission prior to using any images. However, if you do not wish for us to use your images and x-rays in this way please inform us.
